Application Requirements for July 2010 Nurse Licensure Exam

The Professional Regulation Commission will administer the First Phase of the 2010 NURSES Licensure Examinations on July 3-4, 2010. The deadline of filing of application for first time applicants will be on May 14, 2010.

Application will be held at GLOBAL CITY INNOVATIVE COLLEGE (Audio Visual Room) on May 7, 2010, 7:00 AM to 4:00 PM

Please prepare the following:

  1. Original and photocopy of Transcript of Records with scanned pictures and with remarks "For Board Exam Purpose";
  2. Original and photocopy of Birth Certificate from the National Statistics Office n NSO security paper. Those whose NSO copy are not readable must submit a Birth Certificate issued by the Local Civil Registrar of his/her place of birth;
  3. Married female applicants must also submit Marriage Certificate issued by National Statistics Office in NSO security paper;
  4. Original Related Learning Experience duly notarized;
  5. Original OR-DR Cases duly notarized;
  6. Year 2010 Community Tax Certificate;
  7. Four (4) pieces of passport size pictures with complete nametag.
